Incident Type: Motor Vehicle Fatality

Incident Brief:
On December 6, 2012 at approximately 4:05 p.m. the Tomah
Police Department responded to a traffic crash that occurred on N
Superior Ave near Flint Ave in the City of Tomah.
Upon arrival and subsequent investigation of the crash it was
determined that the driver and lone occupant of the vehicle was
Harley Leach. Leach provided information to the officers that
he had been operating his vehicle southbound on Superior Ave when
his eye glasses fell off. Leach’s vehicle traveled into the
median between the north and southbound lanes, back across the
southbound lanes, into the west side ditch, and over the railroad
tracks. The vehicle stopped when it struck a tree on the south
side of the railroad tracks.
Leach did not have any visible injuries that appeared to be life
threatening but complained of chest pain caused during the crash.
Leach was medically transported to the Tomah Memorial Hospital via
the Tomah Area Ambulance Service.
The vehicle sustained severe damage to the front and undercarriage
areas of the vehicle.
The investigation determined that the cause of the crash was Leach’s
failure to have his vehicle under control.
On December 7, 2012 at approximately 7:00
a.m.
the Tomah Police Department responded to a
call of an unresponsive
subject in an apartment within the City of Tomah.
Upon arrival a subject, identified as Harley Leach, was located
deceased. A cause of death was not immediately known.
A subsequent autopsy ordered by the Monroe County Medical Examiner’s
office has determined that Leach died as the result of internal
injuries sustained in the vehicle crash.
